Agreement

I certify that all the information on this form is correct and is submitted for the purpose of attaining credit.  If applicant is making this application on behalf of a company, I further certify that I have the authority to contract, negotiate, and sign for the above said company and any parties making orders on behalf of said company. I expressly agree to make payment in full to Shook Electrical Supply for all purchases in accordance with terms contained in the invoice(s). If the invoice(s) is drafted without terms, the default terms shall be net payment due within 30 days of delivery. I understand that accounts that are overdue and accounts not paid within the agreed term period will be charged a interest rate of 18% per annum, and future orders will be due upon order until the account is brought to a good standing. Further, Applicant shall pay any and all costs and expenses incurred by Shook Electrical Supply in collecting from applicant any past due amounts, including but not limited to reasonable attorney fees, court costs and expenses. Applicant agrees that all claims against Shook Electrical Supply are waived unless presented to Shook Electrical Supply.
